With the mass amounts of eldercare and housing options, it can be overwhelming to sort through all the
information while attempting to decide on the resources that best fit you or your loved one's needs and desires.
At A Positive Choice, we are assisting individuals and families with making eldercare decisions for their loved ones by providing support and advocacy.
Our Lead Case Managers are specialized in assisting members with finding their next best place to call home.
Case Management
Considering the fast track of health care services today and the competitive nature of all Health and Alternative programs A Positive Choice Resource & Referral Services is the members' solution within many areas of housing and health care resources.
COMMUNITY SUPPORTS
Community Supports are new statewide services provided by Medi-Cal managed care plans as cost-effective alternatives to traditional medical services or settings. Community Supports are designed to address social drivers of health (factors in people’s lives that influence their health). These services are building on and scaling existing work in the Whole Person Care Pilots, the Health Homes Program, and Home and Community Based Service Waivers.
Our Lead Case Managers assist with finding members and their loved ones a home filled with independence, socialization, and the stability to live a positive and productive lifestyle with dignity.
TRANSITIONAL ASSISTANCE FOR THE HOMELESS & ECONOMICALLY DISADVANTAGED
Providing a wide range of resources and referrals will allow individuals to live a socially healthy lifestyle within their community. Providing individual members with a personal assistance assessment with a plan of action ensures each member is provided with resources for housing, health care, meal resources, financial planning resource, and referral assistance.
